I confirm that I have been able to review the conditions and the public offer prospectuses of all registered mandatory pension funds and key investor information, if available, and I agree with the terms and conditions of my selected pension fund or pension investment account.
I am aware that by submitting the application for the exchange of pension fund units, the units in the current pension fund shall be redeemed and exchanged for pension fund(s) units of my choice. The hereby application can be amended until the date provided for the submission of an application for the exchange of pension fund units by way of submitting a new application. I am aware that by assigning the proportions of the units to be redeemed in such a manner as to make their sum total less than 100%, the units not submitted for redemption shall remain in their current pension fund. I am aware that by submitting the application for the exchange of pension fund units, the pension fund to which my mandatory funded pension contributions are made or should be made, shall remain the same.
I am aware that by submitting the selection application my mandatory funded pension contributions will be received in the mandatory pension fund or pension investment account chosen above. In order to change the selected mandatory pension fund or pension investment account, a new choice application shall be submitted.
I am aware that the II pillar choice application cannot be withdrawn and I can submit an application to leave the II pillar 10 years after the submission of the choice application or 5 years before the retirement age.
I am aware of the fact that if I submit an application for changing the contribution rate on 30 November at the latest, I undertake to pay the mandatory funded pension contribution on the fees set forth for in the Funded Pensions Act at the rate stated in the application from 1 January of the year following the year in which the application is submitted. I am aware of the fact that I can amend the submitted application until the end of the application period provided for in the Funded Pensions Act by submitting a new application for changing the contribution rate. I am aware of the fact that an application submitted after the end of this period will take effect on 1 January of the year following the next application period.
